Insurance and Fees

  • I am in network with the following insurances:

    If you are unsure of your eligibility, I recommend calling the number on the back of your insurance card to confirm that Alison Fedoris Leslie LCSW (Do and Hope, LLC) is considered as an in-network provider.

  • Out-of-Network Fee and Confirming Benefits

    I am also able to provide a superbill for services conducted if you are out-of-network. If you are out-of-network, individual psychotherapy services (60 minutes) are $120/hour.

    If you are planning to pay out-of-pocket, contact your insurance provider to find out about any out-of-network benefits you may have. I can provide a monthly superbill for potential reimbursement.

  • Cancellation Policy

    I require 24-hours notice for cancellations, otherwise the full fee is expected, unless we are able to reschedule the session within the same week.

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
